Dear Pastor:
Beginning January 1, all Annual Reports will be filed online, using the same username and password your church was assigned for online free subscriptions of Light & Life magazine. Please read all instructions carefully and thoroughly.
Included are the Annual Report materials for reporting your church’s 2011 statistics.

Read the instructions and complete the form. Then:
1. Go to www.fmdata.org. Log-on using your church’s username and password (same as last year). Fill in the appropriate information for your church.
*The information at www.fmdata.org will not only be used for Annual Reports, but also for Light & Life magazine, and the Church Finder, so it is extremely important that your church’s information is kept up-to-date. Please make sure the Senior Pastor is listed as the primary contact and include the church phone # and an e-mail address if available. Also, be sure to fill in the church’s mailing address for Light & Life magazines, as well as the physical address for the Church Finder.
2. It is very important that you provide accurate information and that it be submitted on time.
3. All reports must be completed by January 31, 2012.
